Job Description

PRIMARY RESPONSIBILITIES/FUNCTION: Must have ICU experience, advanced heart failure experience; or Cardiavasular experience.

The Heart Transplant Coordinator (HTC) is responsible for the coordination of required testing, management and follow up across the continuum from evaluation, pre-transplant (waitlist management), during transplant and post-transplant.

The HTC will maintain regulatory integrity and adherence to established regulations throughout all processes. Educate patients, families and other care givers on the transplant process through the continuum. The HTC will have on-call responsibilities for the heart transplant program, which includes collaborating with Gift of Life, medical and surgical teams on the inpatient and outpatient services, all members of the multidisciplinary team and consulting specialists as indicated. The transplant coordinator will also support the outpatient transplant clinics.

ESSENTIAL FUNCTIONS:

  • Participate in outpatient clinic hours, inpatient rounding and the 24-hr/7 days on-call schedule rotation
  • Coordinate the transplantation process when donor organ is accepted including communication with organ procurement organizations, patient/families and multidisciplinary medical team.
  • Coordinate evaluation of potential heart transplant recipients
  • Initiate and provide clinical follow-up and maintain program protocols for patients in all phases of the transplant process.
  • Maintain regulatory requirements for CMS, UNOS and Joint Commission for the transplant program.
  • Provide education to potential transplant recipients through the continuum of care including pre and post-transplant teaching to ensure maximum outcomes.
  • Triages pre and post-transplant outpatient phone calls and communications during regular and off-hours when on call in collaboration with the physician team.
  • Collaborates effectively with all multidisciplinary team members and all care teams/disciplines involved with care of the transplant patient.
  • Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.

  • Develops patient education materials or identifies external resources that are appropriate for the patient education in all phases of transplantation care in collaboration with the multidisciplinary team.
  • Educates nurses and other multidisciplinary care team members including community partners about patient care issues to promote positive outcomes for transplant patients.
  • Provides education and consultation about transplantation and organ donation for the community at large.
  • Serves as a resource person/preceptor in organ transplant support for graduate and undergraduate nursing programs.
  • Communicates effectively to maintain positive working relationships and achieve desired outcomes.
  • Participates in the development and implementation of continuous quality improvement programs.
  • Participates in multidisciplinary transplant meetings, including, but not limited to patient selection committee, rounds and huddles.
  • Attends and participates in program specific quality meetings and performance improvement projects.
  • Maintains education and current working knowledge of policies and procedures unique to the transplant program.
  • Maintains education and current working knowledge of regulatory requirements unique to the transplant program and the role of the transplant coordinator.

EDUCATIONAL/TRAINING REQUIREMENTS:

Bachelors of Science in Nursing Required

Previous critical care and/or CICU/ CVICU/CTICU experience preferred

CERTIFICATES, LICENSES, AND REGISTRATION:

Registered Nurse

CCTC certification within 2 years of becoming eligible to sit for the exam

EXPERIENCE REQUIREMENTS:

Minimum of 5 years nursing experience with at least two (2) years in critical care, transplant, disease specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease specific.
